Wastewater discharge reports.

As a means of determining compliance with this chapter, with applicable SPDES permit conditions, and with applicable state and federal law, each commercial and industrial user shall be required to notify the Chief Plant Operator and/or Superintendent of Sewers of any new or existing discharges to the POTW by submitting a completed commercial/industrial wastewater discharge questionnaire to the Chief Plant Operator and/or Superintendent of Sewers. The Chief Plant Operator and/or Superintendent of Sewers may require any user discharging wastewater into the POTW to file wastewater discharge reports and to supplement such reports as the Chief Plant Operator and/or Superintendent of Sewers deems necessary.

All information shall be furnished by the user in complete cooperation with the Chief Plant Operator and/or Superintendent of Sewers, within 30 days of a request.
